2025 Alumni Service Award Nomination Form

Established in 2024, the Alumni Service Award was created to honor alumni who have demonstrated outstanding loyalty to Parish and made outstanding contributions to promoting the activities and goals of the School through contributions of their time and effort.

The alumni candidate must have made significant contributions to the Parish community through their loyalty, service, and commitment to the mission of the School.

A special award ceremony will be held during the Annual Founders Day Celebration. The Award Recipient will also be recognized with the installation of an alumni panel in the Great Hall on the Midway Campus.

Nominations may be submitted year-round, but the deadline to be eligible for consideration for the 2025 award is April 1, 2025.

The Alumni Board and selection committee (Head of School, Assistant Head of School, and Head of Upper School) reserves the right to request additional information about nominees from the person nominating.
